Interstate Health Papers Form
This form is for traveling out of state with your equine. It needs to be filled out to verify locations and dates of travel. Note: a physical exam needs to be done by a DVM within 10 days of leaving the state (if the horse has NOT been seen within the last year).
A physical exam needs to be done by a DVM within 30 days of leaving the state (if the horse HAS been seen in the last year).
